Support Coordination

At Outback Therapy our Support Coordinators are local community members who are familiar with the supports and services available to Lightning Ridge and surrounding areas. Their role is to support you to navigate the NDIS, connect you with support and ensure that you get the most value out of your plan to meet your individual needs.

What does a Support Coordinator do?
The role of a Support Coordinator is to work with you to ensure the supports you access increase your capacity to live more independently, maintain relationships, manage service delivery tasks and be included in your community.

A Support Coordinator will:
❖ Help you to understand and implement the supports funded in your NDIS plan
❖ Link you to service providers, as well as community and government services you
could use
❖ Assist you to negotiate with service providers about the supports and services
offered, the cost involved and resolve points of conflict
❖ Assist with developing service agreements between you and your providers
❖ Assist you to ensure you are getting the most out of your funded supports within
your plan, in line with your needs and preferences
❖ Assist you to build your ability to exercise choice and control
❖ Support you to build your capacity to act independently so that you are
empowered to coordinate your own supports and, participate in your community
❖ Support you in times of crisis
❖ Assist you in the planning and preparation for your plan review

How do I get Support Coordination?
Not everyone will get support coordination included in their plan. During your first planning meeting or plan review you will need to let your planner or Local Area Coordinator (LAC) know the reasons why you need it. Funding for support coordination is based on what is deemed ‘reasonable and necessary’.
